Tried many WinUAE configs including:

UAE-configuration cachesize 0 cpu_cycle_exact true cycle_exact true cpu_speed real

and NOCACHE WHDLoad tooltype,
There are also random small delays/micro-stutterings.

Never mind: seems it was an emulation issue regarding Picasso emulation (?)
For some reasons switching WinUAE: "Expansions/Hardware Sprite Emulation" off solved all issues.
I've never thought WB Picasso emulation could affect a non-OS-friendly game launched with WHDLoad. Probably it has something to do with sprites registers, who knows...

Issue can be closed.
